How to Apply for Pre-Kindergarten

1Create an account using the following link and add your child's information: https://www.cmsk12.org/Page/372Click 25-26 CMS Enrollment and the Next button to complete an application for your child.3Upload the necessary documents to your dashboard.4You will receive an EMAIL regarding your child's development screening from the Pre-K department.5You will then receive a final EMAIL regarding your child's placement from the Pre-K department to confirm acceptance.
ADDITIONAL DOCUMENTS REQUIRED BY 30TH DAY OF SCHOOL ENROLLMENT
(If your child is placed in the Pre-K Program)- Child's up-to-date Immunization Record
- Child's most current health assessment (on NC form) - completed within 12 months prior to the first day of school.
Required Documents for Pre-K Application Process

1. Proof of date of birth and legal name
ONE (1) of the following documents must be shown:
- Original or Photocopy of Birth Certificate
- Passport
- State-Issued Identification Document
- US Department of State (I-94 Arrival/Departure Record)
- Refugee Resettlement Letter (Local sponsoring agency, US Department of Health and Human Services, Office of Refugee Resettlement)*
- Decree of Adoption
- Student's Driver's License
- Life Insurance Policy
- A certified copy of any medical record of the child's birth issued by the treating physician or the hospital in which the child was born
- A certified copy of a birth certificate issued by a church, mosque, temple, or other religious institution that maintains birth records of its members
- Previously verified school record

2. Proof of ALL current household income
If one or both parents in the household are working, please submit one piece of proof below for each parent in the household:
- Four (4) current consecutive pay stubs (if paid weekly), two (2) current consecutive pay stubs (if paid bi-weekly or semi-monthly), one (1) recent pay stub (if paid monthly)
- 2024 W-2 or 1099
- Tax return (1040 form) for 2024 if submitted after April 15, 2024.
- A letter from the parent's employer that is signed by hand, with a recent date and amount/frequency of pay.
If one or both parents are self-employed
- 1040 Tax form from 2024 Tax return. We can not accept Schedule C.
If neither parent is working, please submit other sources of household income:
- Unemployment/Disability award letter
- SS/SSI award letter
- Child Support statement
**All benefit letters must be dated within the past 12 months of the date you submit your application

3. Current proof of Mecklenburg County residency
ONE (1) of the following documents must be provided:
- Copy of residential deed OR copy of most recent residential mortgage statement
- Notarized Residency Affidavit AND copy of mortgage statement, deed or lease from homeowner/leaseholder affirming tenancy
- Copy of residential lease - including signature page
- HUD closing statement
AND
ONE (1) document from one of the following categories:
- Any ONE utility bill or work order dated within the past 30 days, including gas, water, electric, telephone or cable
- Valid NC Driver's License or Valid NC ID Card
- Dated within the past 30 days:
- Pay Stub
- Bank Statement
- Credit Card Statement
- Current Vehicle Registration
- Dated within the Past Year:
- Vehicle Tax Bill
- Property Tax Bill
- W2
- Medicaid Card
OR
ONE (1) of the following documents must be provided:
- Letter from an approved agency (group home, shelter)
- Refugee resettlement letter
- Copy of Charlotte Housing Authority lease

4. Current Immunization Record (Required within the first 30 days of school)

5. Current Health Assessment (Required with the first 30 days of school)

6. CMS Residency Affidavit (If Applicable)
If you live with someone who owns or rent, you will need to submit a Notarized CMS Residency Affidavit. Click the link below to download a copy of the Residency Affidavit to submit with your application once completed and notarized, along with the owner's lease or mortgage agreement and utility bill.
