Moving within CMS
Please complete the following forms. You may email Kaya Filiz or drop by the office. Please do not send screenshots if emailing. Once you have completed and submitted your change of address documents, please read carefully through the registration information for your child’s specific grade level before completing the course registration card.

Moving outside of CMS
Students who move over the summer and will attend a school outside of CMS in the 2021-22 school year must be withdrawn from school by a parent or legal guardian.
A student will be withdrawn from the school that they would attend for the 2021-22 school year. Students in 8th grade in 2020-21 will withdraw from their assigned CMS high school. Students who were in 5th grade in 2020-21 will be withdrawn from their middle school. Current 6th and 7th graders will withdraw from CHMS.
Students withdrawing from CMS must return their Chromebook and charger to Community House Middle School. Please check the website for school office hours to return your technology.
If you have questions about the withdrawal process or if you need student records to give to a future school, please email Kaya Filiz. Upon registering at your child’s new school, the accepting school will request your records from CHMS. Students who are moving within CMS do not withdraw from school. Please see the Moving Within CMS tab for more information.

Course Card (PLEASE PRINT, COMPLETE AND BRING TO SCHOOL WITH COMPLETED ENROLLMENT DOCUMENTATION)
Course Descriptions (Read the Course Descriptions for information on offered electives. The only electives in which we guarantee placement is band and orchestra. We do however guarantee that your child will be placed in their appropriate math level. Please direct all math questions to Jennifer Weiss. All students will be placed in an English Language Arts, Science, and Social Studies class. North Carolina requires that all students take Physical Education each year of middle school. Please email any questions about the course selection process to your grade level counselor, Valerie Johnson.
PowerSchoolClass Registration Instructions
Parents Mobile Device PowerSchool Instructions: PowerSchool also has a mobile app. It is available to parents and students to download for FREE as a combined parent and student app from the App Store for the iPhone, and iPad, to view your child’s grades and assignments. NOTE: To complete the mobile access you will need the District Code using all CAPS: GJKX. It will then ask for your child’s student ID number, which will be located on their class schedule card.

Schools Requesting Student Records
Please submit all student record requests via email or fax to Kaya Filiz (see contact info above).
*** Important to Note: Parents that are requesting school records for school application must follow the instructions from the admissions office of the school to which they are applying. Schools receiving student records have specific methods in which they accept official student records.
