How to Report a Student Absence

Report an Absence Form

>> On the school's home page, click the Absence Tab, shown by the arrow below, and fill out the form as directed.

To report an absence or early dismissal:

Please email the following information to Jennifer McKinstry - JenniferL.McKinstry@cms.k12.nc.us

Student name and ID number

Parent's name

Reason for absence

Date(s) of absence

Callback phone number

If you would like to request an excused absence for a valid educational opportunity, please fill out the form below and return it to Ms. McKinstry in the front office. The principal will review and give approval if appropriate.
