What is a Blue Card?
Your student is required to have a completed Emergency Locator Card (Blue Card) filed in the McKee Road Main Office. This will help school personnel to quickly locate the emergency contact in case of an emergency.
You will receive this card from your teacher at the beginning of each year. If you start mid-year or do not receive one, please request one from the front office!
Physical Exams/Health Assessments
Starting in 2016-17, parents/guardians must submit proof of a health assessment for each child enrolled in N.C. public schools for the first time. This also applies to each new kindergartner, even if you have previously submitted a pre-k health assessment form. The health assessment must have been completed within 12 months prior to the first day of school.

Immunizations
In compliance with North Carolina law (General Statute 130A-152-157), you must present certification of immunizations at the age required by law on or before the first day your student enters school. Any medical exemption must be in writing from a physician and must state the basis for the exemption. Please call the nurse if you have questions concerning religious exemptions.
Current immunization records are necessary for enrollment for those students new to CMS or who have transferred outside and back into CMS.
Please bring a copy to the school and keep the original for your own records.

Medications at McKee Road
Medication will ONLY be administered when accompanied by a completed Medication Authorization Form signed by both the parent/guardian AND a physician licensed in North Carolina. This is to maintain the safety for all our McKee Road All-Stars!

Parents/guardians must bring the medication to the school office in its original labeled container, do not send medication to school with your student.
Each medication requires a separate form, and a new form is required each school year.
If your student needs medication of any kind (over the counter or prescription) during the school day, you may come to school and give the medication to your child when it is needed or speak with your healthcare provider about giving your child the medication before or after school.
