Absences
Regular and full attendance is essential to a strong academic year and growth for all students. We ask that families please do their best to schedule events and appointments outside of the school calendar and daily hours to ensure our students have full access to instructional time throughout the year. In the event that your child is or will be absent, please notify our Attendance Secretary, Ms. Isabel Velazquez, by email (isabel1.velasquez@cms.k12.nc.us). Please notify us within 48 hours of the absence to help ensure accurate records for your student & our teachers.
Enrolling for the 2024-2025 School Year
Welcome to Crestdale Middle School! Please complete the following forms to enroll your child. You may scan or your completed documents to Ms. Stuart at jessica1.stuart@cms.k12.nc.us and she will contact you regarding your enrollment. Once you have submitted your enrollment documents to Ms. Stuart, a school counselor will contact you regarding your child's registration.
New Enrollment Checklist:

Registration and Enrollment Information
We are excited that your child will be attending our school! Below you will find important information to help you get your child registered properly.
Our registrar, Ms. Stuart, is available to register and withdraw students by appointment only. Please call her at 980-343-5755 to set up your appointment. To help make your appointment go smoother, print out the CMS Student Enrollment Packet (English or Spanish). This will need to be completed, and the required documents will be needed at the time of your registration. Please contact Ms. Stuart with questions.Tdap Vaccine for 7th Graders
Tdap Vaccine and Meningococcal Conjugate Vaccine for all 7th graders - North Carolina state law requires all 7th-grade students to receive one dose of Tdap (tetanus, diphtheria, pertussis) and one dose of meningococcal conjugate vaccine. Your child will need to bring a copy of the shot record to school on the first day of 7th grade.
After the 30th day of school, students who are not in compliance will not be allowed to attend school, per state law. Please contact your healthcare provider or the Mecklenburg County Health Department to schedule an appointment.
Please make sure all immunization records are turned in to the school nurse.
Transferring from Another CMS School
You need to have the following:- Copy of last report card, current class schedule and grades (if applicable), and withdrawal form from the previous school

Address Changes:
CMS requires that you now submit 2 proofs of Mecklenburg County residency when you are changing your address.
- You can use the link above to access the CMS required proofs of residence.
- We CANNOT change your address unless you have the necessary 2 proofs of residence at the time you are requesting an address change.
- All address changes must be completed in person.
- This policy may affect transportation to and from school for your child. Please be advised that you may have to provide transportation for your child temporarily.
Transportation
Eligibility - CMS will provide transportation services for eligible students to and from their residence address. It is important that you maintain an accurate residence address for every student attending school. Address changes are only processed at your assigned school.
If you choose to attend your original lottery assigned school and it is not in your zone, you will not receive transportation. If you change your address after your assignment, you have the option of attending your new home school or your original lottery assigned school. However, if you choose to attend your lottery assigned school and it is not the home school for your new address, you will not receive transportation.
Please note: Conditions such as grandfathered students and/or students who may have relocated from one address to another may establish different transportation eligibility criteria. Individual student situations may vary and not all conditions may apply to the above general statement.
